Payment must be received by the early bird registration deadline of 12:00 noon GMT on 13 January 2012 to qualify for the rate.

All participants must submit a completed registration form, this should be completed online. The 27th International Conference of Alzheimer’s Disease International requires the full data for each individual participant, including updated postal address and email address.

Group Registration

For group registrations, a minimum of 10 participants is required.
Please click here to download the group registration form, once complete please send to MCI UK vy email at adi2012@mci-group.com. An account will then be created in the registration system and the group administrator will then have access to the registrations for tracking and modification purposes. (Please note that groups will not be confirmed until payment has been received.)

Letter of Invitation

If you require a formal letter of invitation for the purpose of a visa application, this will be sent by email only once full payment for your registration has been received. Please request your letter of invitation online when you register for the conference. If you do not have access to email, a letter can be sent by fax to you. Please contact the conference organisers. It is important that you apply for your visa at least 8 weeks prior to the conference to ensure that your Visa is accepted.

Payment

Full payment should be received before the relevant deadline. To facilitate processing, participants should clearly indicate in all remittances their name, address and the purpose for which the payment is made. Either of the following means can be used for payment:

1. Credit Card

Eurocard/MasterCard credit or debit cards, Visa and American Express cards will be accepted. Please fill in your full credit card details on the registration form.

2. Bank Transfer

Confirmation

If you choose to pay by bank transfer your registration will not be valid until full payment has been received including all bank charges. Please send a printed copy of the bank transfer along with the registration form to the organising secretariat at adi2012@mci-group.com. Upon receipt of proof of payment a full confirmation of registration will be sent.

Participants should bring the letter of confirmation to the registration desk at the conference venue when collecting their documents.
